Appointments: All patients MUST register at the front desk upon arrival.

Appointments can be booked in person or over the phone. If you are unable to make your scheduled appointment, please provide the staff with a 24-hours cancellation notice. Missed appointments are subject to charges at the discretion of the physician. 

 

Personal information: Please update any changes to your personal information and health coverages with the front desk at the your earliest convenience.  

If you have any of the following third party insurance coverage, you will need to present the applicable corresponding form at EVERY visit: UHIP, Blue Cross, More Care

 

Pets: Please note that pets are not allowed in the clinic, with the exception of guide dogs for the visually impaired.

 

Behaviour: At CW Health, we strive to treat all patient encounters with the highest degree of professionalism. To maintain this environment, we require all clinic patients to act in a mutually respectful manner. Behaviours that are threatening to staff and other patients may lead to dismissal from the practice.
