Eye Disease Screening Available

Church Wellesley Health Centre has partnered with Euclid Telehealth to offer OHIP covered eye disease testing to our patients. Euclid will be on site at our location on the first Monday of every month for eye disease screening.

Eight million Canadians are living with eye disease and blindness. Annual comprehensive eye assessments provide early detection for eye disease.

Euclid Telehealth focuses on eye disease assessments for the leading causes of vision loss and blindness, such as: glaucoma, diabetic retinopathy, macular degeneration, and others. The healthspan of eyes is their priority and while many eye diseases have no symptoms, Euclid uses leading technology and a trusted network of surgeons and ophthalmologists to identify and prevent eye disease.

Anyone over the age of 18 can access OHIP covered eye disease screening (it is always good to set up a baseline at a younger age).

Who Should Have Annual Eye Screening:

  • Patients over the age of 40 years old

  • Patients who are diabetic

  • Patients with a family history of eye disease

  • Patients who experience any of the following:

    • Blurred vision

    • A gradual change in vision

    • Increased difficulty driving at night

To book an appointment for eye disease screening, please call Euclid Telehealth directly at 1-800-511-5661.

Please note that this is eye disease screening, and not refractions such as nearsightedness, farsightedness, astigmatism or presbyopia for which you would see an optometrist and get glasses.
